ACCESS/VISIT REQUEST INSTRUCTIONS
Contractors and Subcontractors engaged to perform work at Marine Hydraulics International, Inc. must submit a
completed Form MHI-SEC-003.1 to MHI Security via email at visit.request@mhi-shiprepair.com to obtain a
"Visitor's Badge" for their employ`ees as well as the employees of any subcontractors they may employ.

Naval Station Norfolk and Norfolk Naval Shipyard
Work performed at Naval Station Norfolk and Norfolk Naval Shipyard will require a RAPIDGate badge.Information
regarding obtaining a RAPIDGate badge can be found at www.rapidgate.com.
- Please contact our security office to obtain sponsor information to enroll in the RAPIDGate program.
- Work performed at NNSY will require a cover letter on company letterhead in addition to Form MHI-SEC-003.1.
- Cover letter should include: Company logo, address, phone, email, point of contact, Vessel name, dates of visit,
contract/solicitation number,visit classification(Unclassified/Classified), statement verifying all personnel are U.S.
Citizens, printed name, title and signature of requesting authority.

Visitors who have a security clearance that can be verified in JPAS do not have to furnish proof of citizenship.
Visitors who do not have a security clearance that were born outside of the United States must present
appropriate documents at the Pass Office in order to receive a badge

E-Verify Program Screening
All companies who submit visit requests for their employees to be badged for unescorted access to the
Shipyard must screen their employees through E-Verify (formerly known as the Basic Pilot and Employment
Eligibility Verification Program). E-Verify is an Internet-based system operated by the Department
of Homeland Security in partnership with the Social Security Administration that allows participating employers
to electronically verify the employment eligibility of their newly hired employees.
E-Verify is free and is the best means available for determining employment eligibility of new hires and the validity

AIT Orientation Information
Effective January 1, 2011
This Informational package applies to all Third Party contractors hired by the U.S. Government, who wish to gain access to MHI facilities to perform work on a Government vessel. All AIT contractors will follow this standard set of procedures to gain such access and execute work within MHI facilities.
Insurance Requirements
The Subcontractor shall furnish certificates of insurance with the following terms and conditions prior to begining work to Marine Hydraulics International Inc.:
Workmen's Compensation with U.S. Longshoremen's and Harbor Worker's Act Coverage endorsement attached.
Comprehensive General Liability Insurance with limits of liability of at least $1,000,000 combined
single limit of bodily injury and property damage liability, and including premises, operations, products
and completed operations, blanket contractual liability (to cover the liability assumed under the hold
harmless agreement contained in Paragraph No. 9 of this Agreement).
Insurance with at least the following limits of liability:
- $250,000 each person, bodily injury liability
- $500,000 each occurrence, bodily injury liability
