- Download current Visit Request Spreadsheet
- Complete fields for all persons visiting facility
- Email Visit Request Spreadsheet to visit.request@mhi-shiprepair.com
- Attach supporting citizenship documentation for US citizens born outside of the US
- Contact Security office at 757-545-6400 for all Non-US citizen visit requests

- In addition to the above visit request instructions for MHI Midtown Facility,
also download Regional ContractorsSpreadsheet for vessels located at NOB and email to:

Norfolk Naval Shipyard
- In addition to the above visit request instructions for MHI Midtown Facility,
also include a cover letter for vessels located at Norfolk Naval Shipyard
Cover should include:
Company logo, address, phone, email, point of contact, Vessel name, visit dates, purpose of visit,
contract/Solicitation No.
visit classification (Unclassified/Classified),
statement verifying all personnel requesting access are US citizens, printed name,
title and signature of requesting authority
Insurance Requirements (MHI Midtown facility).
Insurance Requirements
- Comprehensive General Liability Insurance with limits of liability of at least $1,000,000 combined
single limit of bodily injury and property damage liability,
and including premises, operations, products and completed operations, blanket contractual liability.
- Workmen’s Compensation with U.S. Longshoremen’s
and Harbor Worker’s Act Coverage endorsement attached.
- Automobile Liability Insurance with at least the following limits of liability:
$250,000 each person, bodily injury liability
$500,000 each occurrence, bodily injury liability
